This transaction allows you to request a replacement Massachusetts ID only if you have one of the new-style Massachusetts ID cards (see images below).

Your replacement Massachusetts ID will be mailed to the address on file.

Please review the form below to ensure you can provide all required information. If you are unable to provide the required information, please contact the Telephone Center.

The fee for this transaction is $15.

You cannot request a replacement Massachusetts ID online if you wish to keep your Social Security Number as your ID number.
